How は意味が広く「どのように・どの状況」等、という意味です。
How are you? は直訳すると 「あなたの状況はどうですか?」つまり日本語の「お元気ですか？」にあたります。
How do you come / go to "b" ? の場合は どのように "b" に来る / どのように "b" 行く という意味になります。
